TRAINING PROGRAM 1

MECHANICS OF RESIDENCY TRAINING

I.        DURATION OF THE COURSE
       
The residency training program in Urology is for four years with annual reappointment to the next higher years.  Basis of reappointment is satisfactory performance in the year just completed and submission of a scientific paper.

II.        WARD TRAINING ACTIVITIES

Residents are trained to be observant and analytical or signs and symptoms of patients, to develop the proper and judicious use of diagnostic approaches, and to arrive at a sound clinical judgment. They learn the correct dosage and proper choice of drugs and to observe its side effects and adverse reactions. They are trained in the preoperative and postoperative care of the patients, and develop the ability to recognize complications and be able to know the measures to prevent or treat complications.

III.        OUT-PATIENT DEPARTMENT TRAINING ACTIVITIES

Residents are trained to screen patients in need of hospitalization or in need of treatment on an out-patient basis. The various stages of the healing process are observed among patients discharged from the ward and the emergency room and followed up in the dispensary.
Residents are trained in outpatient or office urological procedures such as catheterization, urethral sounding, circumcision and cystoscopy including the different Urologic procedures.

IV.        EMERGENCY AND RECEIVING DEPARTMENT TRAINING ACTIVITIES
       
Residents are trained to act judiciously, promptly and decisively when they meet emergency cases. Ability to differentiate serious cases and act on these with dispatch. Continuous exposure to these experiences build up and develop their clinical judgment.
Proper attitudes and relationships with patients and their companions under these very trying situations are likewise developed.
Residents are trained in the classification of medico legal injuries.

V.        OPERATING ROOM ACTIVITIES

Residents are trained or oriented to the various operating room manners and decorum. They practice the principles of asepsis and antisepsis. It is here that they develop their assistive and operative skills including intraoperative decision making.



SKILLS TO BE LEARNED AND DUTIES AND RESPONSIBILITIES


First Year Urology (Junior Resident)

Skills to be Learned:
1.        Polishing of skills learned in general surgery.
2.        Develop skills in the history taking and physical examination of a urologic patient.
3.        Develop skills in the preoperative diagnostic work up of patients
4.        Develop assistive skill in major urological operations.
5.        Develop operative skill in minor and medium operations
6.        Develop diagnostic acumen and judgment in the assessment and management of non traumatic urologic
                       emergencies.
7.        Learn the fundamentals of diagnostic endoscopy (cystoscopy) and urologic imaging procedures.

Duties and Responsibilities of First Year Urology Resident:

1.        Responsible for the preoperative and postoperative care of patients assigned to him under supervision of the
                       more senior resident.
2.        Responsible for the retrieval and filing of all radiologic or imaging plates of urology patients.
3.        Perform all available urologic imaging procedures and other out patient urologic diagnostic procedures like
                       prostatic biopsy, cystometry.
4.        Assist consultants and residents in major operations.
5.        Perform minor to medium urologic operations upon the discretion of the section chief and upon
                       recommendations of the senior urology resident.
6.        Attend to urology outpatient clinics and see new and follow up patients under the supervision of a senior
                       resident.
7.        Responsible for the completion of charts.
8.        Responsible for the completion of the Operations Book
9.        Recognize traumatic and non traumatic urologic emergencies and refer to the senior residents or consultants.
10.        Go on full time rotation in the urology department.
11.        Be able to present case report.

Second Year Urology Resident (Junior Resident)

Skills to be learned:
1.        Strengthen and polish all the skills learned during the first year of urologic training.
2.        Strengthen and polish assistive skills in secondary and tertiary major urologic surgery.
3.        Learn the operative skills in secondary major urologic surgery.
4.        Recognize and manage urologic complications.
5.        To be competent in urologic imaging procedures and learn the application of other diagnostic procedures in
                       urology.
6.        To be able to conduct, research and write a scientific paper.
7.        Responsible for answering traumatic and non traumatic urologic emergencies, initiation of temporary relief and
                       referral to senior resident.
8.        Learn Uropathology and its application.
9.        Learn and apply basics of Urodynamics.

Duties and Responsibilities of Second Year Urology Resident:
1.        Responsible for the preoperative and postoperative management of service patients assigned to him.
2.        Supervise and assist all operations of first year urology and GS rotating residents.
3.        Assist the senior urology resident in major operations and endoscopic surgeries.
4.        Attend to follow up of patients in the out patient department.
5.        Perform medium operations at the discretion of the senior resident and consultant.
6.        Prepare the census and statistics of the department and keeps a record of patients enrolled in specific
                       protocols (Cancer, Trauma, Stone).
7.        Rotate for 3 months in the Department of Pathology (outside rotation).
8.        Perform minor and medium urologic operations.
9.        Learn fundamentals of basic research and animal studies.
10.        Responsible for imparting basic knowledge and skills in urology to rotating interns.


Third Year Urology Resident (Vice chief)

Skills to be learned:
1.        To incorporate skills learned during the first two years of training.
2.        To perform tertiary major open urologic operations with the consultant or senior resident.
3.        To be familiar with endoscopic therapeutic procedures especially TURP, TURBT.
4.        To learn the basics of Endourologic surgery.
5.        To develop administrative skills in the department and hospital.
6.        To learn and develop didactic skills in imparting knowledge and skills to students and colleagues.
7.        To strengthen and polish research skills.
8.        To manage most urologic complications.

Duties and Responsibilities:
1.        Responsible for the supervision of the pre op and post op management of all service patients in conjunction
                       with the senior resident.
2.        Supervise and assist more junior or rotating general surgery or urology resident in performing minor and
                       medium urologic procedures.
3.        Asses out patient urologic patients and admits those in need of in-patient urologic procedures.
4.        Assist senior urology residents in major operations and endoscopic surgeries.
5.        Schedule operations with the approval of the senior resident.
6.        Coordinate and answer interdepartmental referrals.
7.        Perform medium and some major urologic operations with the supervision of the consultant or senior resident.


Fourth Year Urology Resident

Skills to be learned:
1.        To integrate skills learned during the first 3 years of training.
2.        To master tertiary urological operations.
3.        To master urologic endoscopic procedures especially TURP and TURBT.
4.        To be familiar with new techniques in urology namely laparoscopic urologic procedures like ureteroscopy and
                       nephroscopy.
5.        To be familiar with new techniques in urology namely laparoscopic urologic procedures and applications of
                       laser to urology.
6.        To learn from consultants the tricks of the trade in conducting a urologic practice in a private setting.
7.        To improve administrative skills in the department and hospital.
8.        To master the fine art of imparting knowledge and conducting a discussion of a scientific topic on an
                       intellectual level.
9.        To be able to conduct, carry out and supervise research activities personally and on a departmental level.
10.        Learn the basic of Renal Transplantation.

Duties and Responsibilities:
1.        Facilitate Weekly Pre op and Post op, M&.M and other Conferences
2.        Oversee harmonious relation between the medical, paramedical and administrative personnel of hospital
3.        Responsible for specialized urologic equipments
4.        Oversees research output of the department
5.        Conducts lectures, review courses for residents and interns
6.        Responsible to the consultants on all matters concerning training, service and research of the department.
7.        The chief resident is responsible for the care, and management of all service and Eco I patients.
8.        The senior residents are responsible for the care and monitoring of private patients of consultants.
9.        Senior resident is responsible for all urologic outpatients in terms of assessment, evaluation, work up and
                       procedures undertaken in the OPD.
10.        Senior residents are responsible for the supervision of more junior residents in their respective areas.
11.        Senior residents conduct conferences and rounds with junior residents and students.
12.        Rotate at the designated institution for 3 months to learn basics of renal transplantation.
13.        Senior residents refer all preoperative elective and emergency cases to consultants.
